Telehealth Initial Consultation

Once you have completed our “Back on Track” Class online, you can schedule a 1-hour telehealth consultation with our dietitians. The class is a pre-requisite to our dietitian offering you any individual dietary advice. At this telehealth appointment, our dietitian will review your eating patterns, 3-day food log, new patient questionnaire, determine your BMI, calculate your protein needs and make nutritional/lifestyle change recommendations based on what you have been taught at the nutrition class.

Having a consultation with our dietitian allows us to establish you as a patient and offer individual counseling to address your needs. We will look at your medical history, food and exercise logs and review any lab work your medical doctors have had done for deficiencies.

An initial consultation is REQUIRED to establish you as a patient before individual care and dietary advice can be provided.
