In order to participate in these programs, you must be registered by our office for online supervised weight-loss documentation. If you have been registered, please continue.

There are a few important reminders for our online supervised weight documentation program:
- You will automatically receive access to the next month’s content every 30 days from the day you enroll.
- Be sure to save your login information to track progress and ensure you’ll complete the course on time.
It is important that you can have contacted our office to schedule your in-person pre-op nutrition class by now. If you have not, email or call our office immediately to schedule. This class is required for your surgical clearance and fills up quickly.
Aetna & BlueCross BlueShield patients subject to an additional $45 paperwork processing fee, to be billed separately.
